    • Symptoms include:  food sensitivities/intolerance, chronic gas, bloating, constipation, diarrhea, acid reflux, bad breath,  nausea, inflammation, abdominal pain/cramping, etc. 

  • How Nutrition Response Testing can be used to address the root causes of these symptoms and how to restore optimal health. 

  • Digestion transit time test.

  • Cost ~ $50

Initial Consult

Are you ready to start on your Nutrition Response Testing journey towards more vibrant health?

A Nutrition Response Testing Initial Consult is an all-encompassing health assessment to provide you with the first steps in your customized nutrition program.

Your 90-Minute Nutrition Response Testing Initial Consult includes:

  • Comprehensive health history.

  • Body composition assessment. 

  • Digestion Transit Time Test.

  • Explanation of Nutrition Response Testing and complete body scan.

  • Follow-up 90-minute Report of Findings meeting to discuss Initial Consult findings & recommendations for your customized nutrition program.

  • Dietary reference guide.

  • Cost ~ $260 for both Initial Consult AND Report of Findings

Two Ways to Schedule:

If scheduling online, follow steps 1 - 3 for all appointment types:

   1.    Register as a New Patient with The Live Well Center.

   2.   Log in to your Live Well Center account.

   3.  Schedule the type of appointment you want.

If scheduling a 90-Minute NRT Initial Consult please complete Steps 4 & 5 below: 

   4.  Schedule a 90-Minute Report of Findings appointment on a separate day following your Initial Consult.

   5.  Complete the Intake Forms prior to your Initial Consult:

  • General Consent to Treatment

  • HIPPA Privacy Authorization Form

  • Consent to Telehealth Services

  • Medical Questionnaire

  • New Patient Intake & Social History

  • Family History Questionnaire
